New Records Request

Student Information

Please note this form is for students to request records. Third parties, please fax your signed release form to (941) 255-7574 or email to ccps.records@yourcharlotteschools.net.

Please use the name on your birth certificate or if different, your name when you attended.

Please use your contact info, not the institution you are sending records to. This is so records retention can reach you if there is a problem locating your records.

Parents, if your child is over the age of 18, they must be the one to fill out this form. The only exception to this is if you can claim them as a dependent for tax purposes and have the necessary proof.

You will receive an email when processing is complete and your records have been released. Please ensure requestconfirmation@yourcharlotteschools.net is not blocked by your spam filter.

Request Information

Records from the 2020 / 2021 school year and after must be obtained from the last school of attendance.

We do not have copies of high school diplomas. If your employer or college needs a copy of your diploma for proof of graduation, select Official Transcript. For GED requests, please visit the Department of Education Website instead of completing this form. If no one ever claimed your diploma at graduation, check the option below.

If you have lost your diploma and would like it to be reprinted, please visit our website for instructions.

Routing Information

The student whose records are being requested must submit proof of ID if CCPS is sending records anywhere other than a college or university. Doing this now will expedite processing. If the routing info provided is not to a college or university, you will be contacted for proof of ID.

Keep in mind that most colleges will only accept official transcripts that are sent directly by Charlotte County Public Schools.

Due to Florida's open records laws, student records cannot be emailed.

Students requesting pick ups, please allow 48 hours for processing. Hours are 7:30 AM to 4:00 PM Monday through Friday. Records retention is closed most major holidays, please refer to the calendar dropdown for dates we are closed.

Pick up location is at the Charlotte County Public Schools Education Support Building in Murdock.

Submission of this form authorizes Charlotte County Public Schools to release information and/or my student record and confirms I have completed all sections accurately and truthfully, including information verifying my identity. I understand that the recipient of the record(s) will use the indicated document(s) for legitimate interests only and that the information contained therein shall not be further transferred or communicated to any other part or agency without my expressed written consent except under authority of Public Law 93-380, Family Educational Rights and Privacy Act.
